Your GP should have told us that you will be being assisted by a dog on your request form, but in case they have not, please make the booking team aware as soon as possible.
Whilst most of our staff love dogs, not all do and some are afraid of dogs, so to ensure we can make your appointment go as smoothly as possible please let us know in advance.
