Once your GP has identified what test you require, your GP will refer you directly to Physiological Measurements to arrange an appointment for your test.
Your doctor may send your referral via the NHS e-referral system or manually via NHS secure email or fax.
If your GP has referred you for your test via NHS e-referrals you will be given a letter at your appointment containing a unique reference number called URBN.
This letter will also detail your secret password and our telephone number 01691 676496
You will need to phone our Patient Management Centre on 01691 676496 to register the referral – You will be asked for your URBN and secret password during the call.
Once your referral has been located on the system, we will send your referral for clinical triage. We will then give you a call back within 2-5 working days to arrange a convenient day and time for you to attend your test.
If your GP has referred you via paper referral (secure NHS e-mail/fax) then you do not have to contact us as you will be contacted directly by us (Physiological Measurements) to arrange your appointment.
