Who Are We

We are Physiological Measurements Ltd, a provider of community based diagnostics and distributor of related medical devices.  We provide services in partnership with the NHS, to private hospitals and private customers (non-NHS).  We are a Limited company, registered in England and Wales 05478605.  Our registered address is The Old Malt House, Willow Street, Oswestry, Shropshire, SY11 1AJ

What type of information is collected from you?

The personal information we collect might include:

  • your name,
  • address,
  • email address,
  • mobile phone number,
  • information about your health (including any medical condition and medical diagnosis)
  • information about your race or ethnicity.

Why we are collecting your information?

NHS and Private Healthcare/Sports Cardiology Screening
We may use your information to:
  • Process a referral for a diagnostic test we have received from your GP/club doctor/club physiotherapist/private doctor
  • Send you information about your appointment in our clinics or for feedback on your experience
  • Provide the performance/quality information required by commissioners as part of our NHS Contract

Employees, Workers and Contractors
We may use your information to:
  • To carry out our obligations arising from any contracts entered into by you and us;
  • To carry out our obligations under employment, social security or social protection law, or a collective agreement

Delegates Information for Marketing Purposes:
For those delegates or individuals who provide their contact details at PML associated/attended conferences or events, we may also use your information to keep you informed about relevant upcoming events, opportunities within the organisation, and other pertinent updates. This may include retaining your contact information to notify you of relevant seminars, conferences, workshops, or other activities that may be of interest to you. We believe that keeping you informed about such opportunities aligns with our commitment to providing comprehensive support and resources and the purpose for which you provided your details. If you prefer not to receive such marketing communications, you can opt-out at any time by contacting us using the details provided in this privacy notice.
You will also be able to opt out via a dedicated link included on any PML issued correspondence.

Who has access to your information?

We will not sell or rent your information to third parties.

We will not share your information with third parties for marketing purposes.

We may pass your information to our third party service providers, subcontractors, customers and other associated organisations for the purposes of completing tasks and providing services to you on our behalf (for example to send you’re a text reminder of your appointment). However, when we do, we disclose only the personal information that is necessary to deliver the service and we have a contracts in place that requires them to keep your information secure and not to use it for their own direct marketing purposes. Please be reassured that we will not release your information to third parties unless you have requested us to do so, it is necessary for the reasons of medical diagnosis/management of health service or we are required to do so by law, for example, by a court order or for the purposes of prevention of fraud or other crime.

How do we keep your data safe?

When you give us personal information, we take steps to ensure that it’s treated securely. We have put in place measures to protect the security of your information. Third parties will only process your personal information on our instructions and where they have agreed to treat the information confidentially and to keep it secure. We have put in place appropriate security measures to prevent your personal information from being accidentally lost, used or accessed in an unauthorised way, altered or disclosed. In addition, we limit access to your personal information to those employees, agents, contractors and other third parties who have a business need to know. They will only process your personal information on our instructions and they are subject to a duty of confidentiality.

We are Cyber Essentials compliant. This means we adopt good practice in information security. It includes an assurance framework and a simple set of security controls to protect information from threats coming from the internet.

 

How can you access and update your information

PML take the protection of data very seriously. To ensure our ongoing compliance with GDPR, PML have an outsourced Privacy Lead in place who ensures all legal and regulatory requirements are met.

To support the protection of our data we have a number of policies in place which are audited annually.

The accuracy of your information is important to us. We’re working on ways to make it easier for you to review and correct the information that we hold about you. In the meantime, if any of your personal details change, please contact the Patient Management Centre.

You also have the right to ask for a copy of the information PML hold about you (This is called a Subject Access Request). 

How to make a Subject Access Request

Download the subject access request form, fill it in and return it to us, along with copies of information that confirms your identity. Please do not send in any original copies of documents.

You can apply as a third party for someone that you are responsible for, including for a child. You will need to provide proof that you are allowed to act on their behalf.

Your Rights and Opt Out information

In the limited circumstances where you may have provided your consent to the collection, processing and transfer of your personal information for a specific purpose, you have the right to withdraw your consent for that specific processing at any time. To withdraw your consent, please contact the Privacy Lead in writing at privacylead@pml.tel, The Data Protection Office, Physiological Measurements Ltd, the Old Malthouse, Willow street, Oswestry, SY11 1AJ. Once we have received notification that you have withdrawn your consent, we will no longer process your information for the purpose or purposes you originally agreed to, unless we have another legitimate basis for doing so in law.

Transferring your information outside of Europe

We do not transfer any information outside of the European Union (EU)
